NHS Health Check
The NHS Health Check is a free check-up of your overall health. It can tell you whether you're at higher risk of getting certain health problems, such as:
- heart disease
- diabetes
- kidney disease
- stroke
During the check-up, you'll also be told about symptoms of dementia to look out for.
This is available to patients aged 40 to 74, and those patients who do not have a pre-exisiting health condition.

When you are invited
We will send out invitations to eligible patients, and patients who do wish to have an NHS Health Check should contact the surgery to arrange an appointment with one of our Health Care Assistants. This will be a 20 minute appointment.

After your appointment
We'll send you a letter with your health check results and what they mean for you.
